is this hole in the head?

Hey guys, I have a discus which has some markings on its face, I at first paid no attention to it cuz I thought hith was caused by poor water quality, and that wasn't an issue. The spot hasn't grown, it has remained the same size, but it hasn't gone away, could it just be a scrape or something? My readings are 0 ammonia and nitrite, and 5 ppm nitrate. Here is a picture:
