Can someone identify the issue and help me with medication...

Also there are holes on either side just above lips
This was one the most active discus in my aquarium. I also use to pet him during water changes. He has now turned dark with patches and he is hardly eating.

Nothing new was added to the tank. Its a bare bottom 40 gallon tank with 4 discus. Running for more than a year with same fish. I change 70-80 % water every day. Ammonia 0 PH 7.4 Nitrite 0 Nitrate 5. I have now raised temperature to 30C.
Filtration: I have a top filter with bio balls and ceramic rings plus a DIY sponge filter with attached bottle with ceramic rings.
Feeding now reduced to twice a day, goat heart mix in morning and Tetra bits or DBW in the evening before water change.

Darshan this discus does not look good. it may be losing its slime coat due to an irritant (chlorine, nitrite, ph burn) in the water. I suggest salt, or a salt dip immediately.

Also please drop the temp. This looks bacterial and the extra heat is not good in this situation.

Darshan definitely listen to Pat, if she suspects Bacteria then take her advice over mine. The salt will still help but if bacterial, you need real medication. Do you have any on hand?

Thanks Pat and Jacklyn for your suggestions. I have reduced temp to 28C now and gave him salt dip.
Should I continue Salt dip? I have BeFuran, PP and general antibiotics. Any other medication suggestion?

While impossible to tell, it has a Costia look to it to me. Whatever you do, don’t use copper - discus don’t respond well.
You can use salt for extended periods in low concentrations, but research the dose first.
DBW sterile?

Thanks for you suggestion. I wont use copper.
DBW I use are Freeze Dried Australian black worms. I guess most of us feed these.

Freeze drying does not kill (all) bacteria, so just saying that it a potential source that can’t be ruled out.

Stop feeding meat products. Proteins from plant derived products ok,, meat ,,,not ok. And if it were me, imo, I'd be only changing 30 to 50% a day and treating with some API general cure.

Hi Darshan, I would treat with Furan 2 at this point. You can add salt to help.

Hi Matt, Al who is the owner of SimplyDiscus and the seller of Australian freezed dried black worms feel that they are safe. He has worked closely with the farm and trusts the process used to freeze dry the worms.

Sorry I came upon that just now. Pat is correct. Furan 2 is the medication of choice. The fish is in very bad shape at this point so cross your fingers that you have caught it in time.

Hey guys thanks a lot for all your suggestions. I treated my fish with paracidol fw and he bounced back :D
He is now back on the top of pecking order and active as before.
