I hate to burst Bill's bubble, but those actually turn up more often than you realize.
I've seen angels and discus with giant dents in their heads and bodies like that. It's some kind of birth defect rather than an injury. The fry just develop that way, and oddly enough it doesn't seem to have any bearing on their ability to eat or swim around. Some people call them unicorn or rhino discus.

I've had quite a few Heros and Heroina fry end up that way, but these are fish that have 200 - 400 fry per spawn, so maybe they appear more often than discus. Usually it's noticeable by the time they're a few weeks old and I just cull for it. They become snacks for larger fish.
