Injury or Hole in head?

This fish was fine yesterday afternoon. Yesterday evening it was hiding, but I didn't think anything about it. When I got home from work today I noticed that it has this place on it's forehead that was NOT there yesterday afternoon.

Is this an injury or hole in the head disease? Does HIH set in that quickly?

That looks petruding to me and like a wound. Keep up with big daily changes and give it A week

I have it in a QT tank now. Treating with anti parasite and anti fungal meds. Hopefully just an injury that will heal.
