    Hello hopefully somebody can point me in the right direction.


    DISEASE QUESTIONNAIRE


    Problem

    1. Please explain the problems with your fish. When did you notice the problems and did anything unusual happen that you think started them

    When i first got the fish, along with 2 others it was the only one that didn't eat and seemed breathing heavily


    2. Symptoms (i.e. turning dark, excess slime, not eating, clamped fins, flashing, darting, clamped gills, white/yellow/green poop, hiding, headstanding or tailstanding, white on tips of fins, rotting or fungus, blisters/white zits on fish, bloated, cloudy eyes, wounds).

    Right gill is clamped


    3. What medications/ treatments have you already tried and what were the results. Include dosage and duration of treatment.

    Treated it with metro(water column) during the first week, then mixed with food after i noticed it starting to nibble.


    Tank/Water

    4. Tank size and ages, numbers and sizes of fish.

    Three year established planted tank. 8 discus(about a month) 14 Rummy nosed tetras 1 year, 8 cherry barbs about 6 months, 1 SAE 2 years, 1 clown pleco 2 years, 1 bristled nosed pleco 1 year


    5. Water change regime (What percentage and how often).

    50%-60% every 4 days


    6. How long has tank been running? Is it bare bottom? If you have substrate, what type and how deep is it?

    Tank is about 3 years old. Substrate is miracle gro potting soil capped with 3" of topfin sand


    7. Do you age your water? If you do for how long and what is the ph swing.

    No


    8. Parameters and water source;

    Note: Water Parameters are important in diagnosing problems within a tank. If you don't own test kits for the following information, you can purchase them, test your parameters and post this info as soon as possible.


    - temp _85__

    What type of water or combinations of water sources do you use? If it is an RO/tap/well water mix, please list percentages in the mix.

    9. Any new fish, plants or inverts added recently.

    Only the discus


    10. Please tell us what you feed your fish and how often. This can be critical information for solving the problem so be as specific as you can.

    Hikari bio gold pellets, seachem discus flakes and hikari freeze dried blood worm( as treats)



    11. Include any pictures or videos you have which shows the symptoms. If you can't add them to this post, please provide a link to them.

    It would also help if you posted tank size and what you use for a conditioner?And how often is the substrate cleaned?How old/big are the discus?I ask this because it is a planted tank with a very thick substrate.

    Thanks for replying. Ron Waddel pmed me a few days after i posted. I already had it in quarantine by the time he sent me a message. Anyway i think the fish is just deformed, i must've missed it when i picked it up from the dealer. It seems to be eating again. I treated it with API general cure for 3 days. Now it's just salt added to QT water and 80% WC every 2 days. I'll move it back to the main tank when the( i quarantined two fish that were not eating) other fish that is not eating starts to eat.
