Your nitrate reading of 25 is one definite indicator you are not changing enough water. That is based the immeasurables. Definitely should increase wc frequency and get those nitrates to 10 or below

I sift my sand while I do my water change (75% twice a week)
PB seemed to get better so I put him back in my display tank, and he's still not good

I think I have a bigger infection going on in my display

I'm going to start a new thread.

Default Infection in my Main Display Tank

Hi all (I'm back), Let me give you my tank stats:
    125 gallon
    fluval fx6 canister
9 adult Discus, nerite snails, 11 neon tetra, trumpet snails
    85F
75% water change twice a week, I also sift my sand while I do the water change
    ph:8.2
    nitrates: 5ppm
    nitrites and ammonia: 0ppm

So I made a HUGE mistake when I got into Discus 5 months ago: Mixed discus (good quality, not stunted) of different sources with out QT AND added new discus without QT. A few died from stress (I had some fast moving community fish previously with them-they are gone now)
I guess cross contamination is showing up now.

I also lost one of my discus to hexamita ( my assumption, because he had pinkish/white chunky poo and had head down)
Now I think the hexamita has spread because one of the discus stopped eating, and the other is pooing the same poo.
And a few other discus have stopped eating. My Pigeon Blood is very thin.IMG_4481.jpgIMG_4485.jpg

I do believe there could be another infection going on in my tank.

People have suggested to me : aquarium salt, stress zyme plus, and Metro along with high heat (90F)

It will be expensive to treat my main tank But I will do it !

I would get some Metro and treat for at least 10-14 days. You can up the temp to about 86 or so.I would also treat the entire tank.Salt is not needed for Hex.Also keep an eye on that red discus, not sure if they are pimples or ich,but looks like pimples, which is a water issue. If it's ich the higher temp should kill it.Fish should start eating in 3-4 days of treatment. If you can get some freeze dried worms and soak some metro in it that will help speed things along. If you purchase Seachem use a bit more cause it's 70%.

I merged the new one and this one Natalia , you are really dealing with the same issues here..

If you to solve these issues you need to start at the basics, and answer all the questions. You had a fish that was not eating, the PB, but you put him back in the main tank? You didn't answer my questions on treating him, or on what you feed.

I think you need to start here with levamisole... The yellow in the feces is usually an indicator or capillaria eggs...aka worms. Deworming is the easiest thing to try first.. 3 treatments, each at 24 hours followed by large water changes... treat day 1, day 7 day 30 ( or close to that)

I raised the temp to 88, going to raise it to 90 F now
doing 30% daily water changes so far.

The red melon cleared up immediately.

    ok so start with levamisole
    then metro?

blue lagoon thank you for your compliment

    Sorry I forgot to update,

I put him back in the main tank because I was going to treat the entire tank.

    Update:
last night my driftwood fell and scratched my red turq a bit.

Today I woke up and one of his gills look a little white?

I dont know if those two events are related or not.
