blockage.
Take a look if the forehead and pinch stomach
it might be wasting away disease.

Thanks Phil. Once I have a definitive course of treatment I am going to get the guy into a hospital tank and will up the water changes. I think my current regime is the right balance for adults in my tank knowing all the variables. The tank goes up a steady 5ppm a day and I clean the prefilter sponge and my filter socks daily. The fish are all very active, spawning, ect except for the sick one.

From the symptoms I would have thought metro + kanamycin, but you have already tried that.
Can you get a pic that shows the lumpiness?
Its no fun guessing at things like this... a microscope exam of the poop can quickly rule out one culprit and confirm another.
Hi Dan, thanks for your input. I do not have access to a microscope unfortunately. I am going to see if I can find one by asking my neighborhood on Nextdoor to borrow one.

I am pretty sure that’s just yellow pigment on my neon sapphire’s fins from eating tetrabits w color enhancer. Tail fin has the same coloration
I am at a loss on what to do next, I fear that he will just suffer and never get better. I had another discus that showed similar symptoms and by the time the eyes were cloudy there was no curing him no matter what I did. I did not try the heat treatment with the metro because I think there is already a secondary bacterial infection present. The epsom salt has not helped. No other courses of treatment have been suggested and I am positive that a daily water change will not turn this around.
I am considering euthanizing the fish but am having a hard time bringing myself to do it because he is still nice and fat at the forehead despite not eating for at least three weeks.
Any thoughts on a last ditch treatment or opinions on euthanizing are welcome. I am experienced with clove oil euthanasia so I don't need any suggestions on method.

How about a general cure, the api formula... or I have used paraguard in the past as well...
Is he still with the group or isolated?
