Is that a raised area in the center of the body. Not really sure what I am looking at. Are you using a de-clhorintor when doing a water change? If it seems bacterial best to lower the temp to 28.

My discus is getting this on his/her body. It's getting weak and just stay on the same place. I've been keeping her for about a month now.
This is actually the second case, the first case just the same body spots and died very quickly (from sick to dead was about 2 days). They both came from the same fish store.
Water test results: https://photos.app.goo.gl/eoc8bP0cvMS402553
I tested it right after taking her out for quarantine. It's hard to see but overall just the Cl2 is high. Could this be a chlorine issue?
Some tank info:
~400G (1200L) including refugium.
water temp: kept at constant 30 celsius.
water change: 3-4 days.
diet: bloodworm/artemia

Your discus are talking to you....are you listening
It’s to remove chlorine from your water before you put it in the tank. it can be deadly to fish.

I understand its purpose, I mean where could I get those? Amazon? What brand should I use? Would love to see multiple options to choose from.
But it's actually urgent to cure the fish first then fixing the tank. What should I do?
If it is just an external injury, you could apply betadine to the affected area while out of the tank. Then, clean de-chlorinated water. You could also use non iodized salt @ 2tbspns per gallon.
