Water is 86 degress, pH is about 7.8, hardess less than 5 degrees, nitrates and nitrates 0

Please fill out the entire questionnaire (link above) and repost with more details about your tank. Can you get some closer shots of the forehead? 5 days is very fast progression. Do you have a quarantine tank?

Sorry for the delay here. Thats not hole in the head as commonly known but if you do nothing it will rupture and leave a huge hole. its a bacterial infection. By the looks of it its systemic.Im afraid the prognosis isnt good but I have seen at least one case where the fish survived and healed but had scars.
If you want try and save it, kanamycin or oxytetracycline are your best bets.
