I would separate it in a 10 gallon QT tank and try with salt treatment (one level TBS per gallon )for a couple of days first .
1. Please explain the problems with your fish.
This guy started getting dark about a week ago. He eats fine, poops dark, and swims good. I noticed some fin fray around a week as well. Two days ago I noticed the white patch on the forehead area.
2. Symptoms
Turning dark, but will almost instantly return to normal color when its feeding time or when I walk up next to the tank. As mentioned above, the back fin isn't smooth edged like th other fish in the tank and two days ago i noticed the white patch (see pics below) on the forehead area.

110 gallon 2 years old bare bottom. 9 discus ranging from 3-6"
5. Water change regime (What percentage and how often).
30-50% 4-6x/week
6. How long has tank been running? Is it bare bottom? If you have substrate, what type and how deep is it?

7. Do you age your water? If you do for how long and what is the ph swing.

Discus Hans flake small feeding 2x/day and beefheart mix 2x/day
