I would try using Kanamycin. It's the best antibiotic for internal issues. I always resist culling if is can be saved, especially on such a nice discus. Others will disagree. This does seem to be more spread out than just the swim bladder alone. I would treat in a hospital tank.

Who knows, a good dose of Kanamycin and some clean water may turn this nice discus around...
