Jim I don’t believe your media is a problem. My matrix and bio-home is between 3-6yrs old. All I do is rinse it when doing water changes. As to why your fish died it could be anything. That divot may have had something to do with it. The other could have died because the first was gone. I had a pair of angels for years. One night the female tore up the males fins terrible. I had to remove him and treat him with salt. Within a week for no reason the female died. I took it for loneliness. Fish are strange.
