Hi,
Can someone please help me identify this spot and how to treat it. I’m not sure if it’s a parasite or just a wound. It has a slight bulge;swelling, other wise the discus is fine and active.

Your discus are talking to you....are you listening
You need to put this fish in QT with 5tbs aquarium salt per 10G of water and start a treatment regiment of nitrofurazone. If you use furan 2 you will need to double the dose.
This bacteria is very fast moving and needs to be treated quickly.

It looks more like an injury to me, perhaps darted into the drift wood. I encourage you to fill out the disease questionnaire as Pat suggested. If it were me I would use 5-7tbsp salt per 10 gals, and do daily 50% water changes for a week replenishing the salt...but not sure if your plants will tolerate that though...
I also recommend the salt. I have treated discus with as much as 1 tbls per gallon but for this one the 5 tbls per 10 gallons seems reasonable. Discus seem to respond quickly to salt treatments so this is a good first treatment.
Fish has a cloudy eye as well. Please fill out the form so that we can help you.

Update:
Discus is doing a lot better and spot has almost completely disappeared and so is the cloudy eyes.
This is almost 5 days of API General Cure treatment. I will just leave it in the QT for a couple of more days and back to the display tank.9Bcp-u16ROmsEdO6A9vkYQ.jpg
Thanks for everyone’s reply and concern. Highly appreciated.
