I took a look at the offerings and they vary per item...some offer XXXl...I saw some t-shirts and long sleeve shirts for instance that do.I have no control over the availibility as the items are designated by cafe press. It might be worth trying to contact them directly if you have special needs.

Just an fyi....The women's sizes run really really small.
The thing I really like about cafepress is their warranty and customer satisfaction terms..If you aren't happy with a shirt, return it and get one thats a better fit.
