Eye problem

Hello All, I am new to this forum so please let me know of any procedural transgressions. I have a discus with an eye problem. The eye appears to be getting eaten away by something. Check out the pic.

Please fill out and post the disease questionnaire so we can help you better. Whats your water change regime? Water params? Water temp? How big is your tank? Any other fish in there? I can tell you that fish is very dark and doesnt look healthy.
