This depends, 4 weeks is good with an additional 2 weeks of observation by adding 1 fish from your existing tank to the QT tank. ;)

Some experienced people do preventative treatment.

i don't mean to hijack the thread, but is it ok to qt one discus by itself? in a 20 gallon tank? or will it stress the discus out to much?

Also, do you guys typically keep your QT tanks running all the time even with no fish in it? Or do you prepare it when you need it? I assume you'd want to keep the filters running so the good bacteria wouldn't die, right?

I QT on the long side also, usually for 3 months, sometimes just grow out the new guys on their own anyhow--tend to buy a bunch at a time if I am doing it at all. I would always bring one fish from my collection to put into the QT tank for a couple weeks as the last step to ensure success.

One discus alone is a stressed out discus.

I can vouch for that, we had one on his own in the hospital tank for 4 week on his own after taking the others away. While he got better thanks to members on the forum he spent most of the time sulking in corners or under the heater. In the end we took the plunge and put him in the main tank just over a week ago. What a change, he instantly started mixing with the others and now his colour has returned fully
