I have a wounded fish in my tank, but I don't know if he has a disease or he scratched himself by a rock/piece of wood?? Can someone please advice me what to do? See below the wounded fish:

That looks like a simple scrape to me. If you have a good water change routine I'd expect it to be gone in a couple of days. Increasing your WCs for a couple days wouldn't hurt.

My discus have scraped themselves on the wood in my tank on more than one occasion and that is what it looked like. It is no big deal and will heal just fine as long as your tank is well maintained and you do regular water changes. If you are not doing water changes daily I would recommend that you do at least until this is healed.

just happened to my BD last week...i will be all back to normal in a week....so it is back to normal now. dont worry to much unless you see white stuff growing on it.

No need for any stress coat, discus produce plenty of slime coat naturally...it'll be fine.

Didn't know discus healed scars with wcs, learn new stuff everyday here. :)

They'll heal with or without water changes. The cleaner the water, the less likely an infection can occur. But the fish in the picture merely rubbed the slime coat. If it was a gash or deep scrape, it would be wise to keep the water super clean.
