It seems that mostly all my discus fry pass away on the day they become 3 weeks old in the next 3 days. The water is always clean.

You have actually provided no information that may help hone in to identification of the problem.
What can you tell us about the tank the fry were born into?
Have you ever treated the parents for flukes or intestinal parasites?
What do you feed them and how often?
What does "the water is always clean" actually mean in terms of your parameters?
What is the water maintenance schedule for the fry?

Were any of the fry dark, spinning and cramp fins. What is your maintenance schedule and the amount of water change? How long were the fry with the parents? Feeding any bbs?Comment
