Sick Discus

Help!

I put discus together from two tanks. I kept them separate for three months. However, when I put them together after four says I had problems. The fish are at top and some of them are not eating except a few. Listless and one of them has it's head pointed towards the top struggling to swim normal. Just seem lethargic. Any suggestions???
